IT IS THE RESPONSIBILITY OF THIS TEAM TO MAKE SURE THAT THIS EXHIBITION TEAM IS USA HOCKEY REGISTERED. THE TRAVELING TEAM IS RESPONSIBLE FOR PROVIDING PROOF OF USA HOCKEY MEMBERSHIP AND PROOF OF AGE FOR ALL SKATERS, IF REQUIRED.

Important Information

This form must be submitted no less than ten (10) days prior to the Tournament. The form is automatically sent to the Tier II Commissioner.

Scoresheets must be mailed within three (3) days. It is the home club's (CAHA Club only) responsibility to mail the original scoresheet.
